Commercial Business License

Complete this form and bring to City Hall along with a picture ID. There is a $25 fee. This license is renewed annualy. Cost is based on gross revenue. 

Citizen Complaint Form

File a citizen complaint by using this form. Email for to info@cityofcottondale.net    All complaints are reviewed by the City Commissioners.
